Is Pelvic Floor Physiotherapy Covered by Insurance in Ontario?

Does your extended health plan include this type of physiotherapy? Do you have to pay upfront and submit a claim yourself? The short version: most extended health benefit plans cover pelvic floor physiotherapy under their physiotherapy benefit, and InHealth offers direct billing so most patients pay only a co-payment or nothing at the time of their appointment. OHIP covers physiotherapy only in specific circumstances that apply to a minority of adults.

How OHIP Physiotherapy Coverage Actually Works in Ontario
OHIP: the Ontario Health Insurance Plan does not cover physiotherapy for most working-age adults. This is a common source of confusion, partly because physiotherapy was more broadly covered under OHIP before a 1994 policy change.
OHIP currently covers physiotherapy services only for the following four groups:
Children and youth under 19 years of age
Adults aged 65 and older, at OHIP-funded physiotherapy clinics (not all clinics are funded; you must attend a designated facility)
Recipients of Ontario Works (OW) or Ontario Disability Support Programme (ODSP)
Adults discharged from hospital within the preceding 12 months for a condition that required physiotherapy as part of their recovery
If you do not fall into one of these four categories, OHIP will not cover your physiotherapy sessions — including pelvic floor physiotherapy. The good news is that extended health benefits usually do.
Extended Health Plans: What "Physiotherapy" Coverage Means for Pelvic Floor Sessions
Most group extended health benefit plans in Canada include a physiotherapy benefit — typically expressed as an annual dollar maximum (for example, $500 per year) and sometimes a per-visit limit (for example, $60 per visit).
Pelvic floor physiotherapy is physiotherapy. It is not a separate category, a specialty billing code, or an excluded service under most plans. When your plan covers "physiotherapy," that coverage applies to your pelvic floor sessions under the same annual pool.
The best way to know your specific coverage is to call the member services number on the back of your benefits card and ask: "Does my plan cover physiotherapy, and is there an annual maximum or per-visit limit?" The call takes five minutes and removes all uncertainty.
What Information to Give Your Insurance Company When You Call
When you call to verify your physiotherapy coverage, have the following ready:
Your group plan number and member/certificate number (on your benefits card)
Your date of birth
Ask specifically: "Is physiotherapy covered, what is my annual maximum, is there a per-visit limit, and do I need a physician's referral for reimbursement?"
Note: you do not need a referral to book physiotherapy in Ontario. But some insurance plans require a physician's referral to reimburse claims — even if the clinic does not require one to book your appointment. If your plan requires a referral for reimbursement and you do not have one, you can still book and attend your appointment; you will simply need to submit claims manually rather than using direct billing.

Direct billing means InHealth submits your insurance claim on your behalf at the time of your appointment. You pay only the co-payment, the shortfall above your per-visit maximum, or nothing at all if your benefit covers the full session fee.
To set up direct billing at your first appointment, bring:
Your insurance card or the plan information (group number, member ID, provider name)
Your date of birth
The name of the primary plan holder if the coverage is through a spouse or partner's plan

When Direct Billing Is Not Available
Some group benefit plans — particularly older self-insured plans, or plans through smaller employers — do not accept electronic direct billing from physiotherapy providers. In these cases, InHealth provides a detailed receipt at the end of your appointment that includes all the information your insurer needs for a manual reimbursement claim. You submit the receipt to your insurer (usually through their app or online portal), and they reimburse you directly.
Motor Vehicle Accident and WSIB Coverage for Pelvic Floor Conditions
If your pelvic floor symptoms are related to a motor vehicle accident (MVA) for example, pelvic pain or incontinence following a collision your treatment may be covered under the Statutory Accident Benefits Schedule (SABS) through your auto insurer, with no co-payment required.
Similarly, if your symptoms are related to a workplace injury including certain musculoskeletal or traumatic conditions affecting the pelvic region, WSIB (the Workplace Safety and Insurance Board) may cover your physiotherapy in full. InHealth bills both MVA and WSIB directly.
How to Maximize Your Annual Physiotherapy Benefit
A few practical strategies:
Book your assessment early in your benefit year so you have the full annual maximum available
Ask InHealth for a receipt after every visit — even if we bill directly, you may want to track your usage against your plan maximum
Confirm whether your partner's plan has physiotherapy coverage — if both plans apply, coordinating benefits (billing your plan first, then your partner's plan for any remaining amount) can reduce or eliminate your out-of-pocket cost
